What Is SOURCE?
The Service Options Using Resources in a Community Environment program is a case management program for Medicaid beneficiaries. SOURCE works to improve the health outcomes of members with chronic health conditions by using case management to connect them with approved health services in their home or community.
Atlanta Pro Health is dedicated to coordinating services for their members. Our case management team works with members, caregivers, physicians, and approved providers to design safe and member-focused care plans.
Atlanta Pro Health will help eligible members apply for the SOURCE program.
Eligibility and Criteria:
- Medicaid benefits
- Qualified income
- 65 or older with a chronic medical condition OR under 65 and physically disabled
- Must meet intermediate Nursing Home level of care criteria
Target Conditions include:
- Dementia
- Alzheimer’s Disease
- Stroke
- Diabetes
- Hypertension
- Lung disease
- Arthritis (Rheumatoid/Osteoarthritis)
- Heart disease
Community Services available through SOURCE are:
- Personal Support Services (personal care, light chore services)
- Adult Day Health Care
- Home Delivered Meals
- Emergency Response System 24/7
- Home Health Services
- Alternative Living Services
- Respite Care
