Home Health Care
When your loved one needs health care at home, we coordinate with home health providers and your physicians so care is arranged around your loved one’s needs.
Home Care Services
Finding the right home care can be tricky. You want someone who is qualified and experienced, and someone you can trust. We help you find the right fit for your loved one’s care.
Our team can help you compare your options and choose a home care provider that fits your loved one’s needs.
SOURCE
SOURCE is a Georgia program for seniors and people with disabilities who need help to keep living at home. Our case managers coordinate SOURCE services so you can access the support available to you.
Elderly and Disabled Waiver Program (EDWP)
The Elderly and Disabled Waiver Program helps Georgians live in their home or community. EDWP provides long-term health services for seniors and people who are functionally impaired, with the goal of decreasing nursing home placement and unnecessary hospital stays.
Eligibility depends on your Medicaid status and your care needs. Call us to find out if you or your loved one qualifies. Our team can help you request an assessment and walk you through the next steps.

What to Expect From Your Assessment
During your assessment, we ask about your loved one’s health and daily needs. We then explain the programs and services that may fit, including EDWP, SOURCE, and home care.
