We understand how complex it can be to access quality healthcare, and we want to help. Our care management services are designed to be member-centered, family-focused, and culturally competent. We can help you understand and manage your health condition, coordinate services, and locate community resources. Our staff makes health care easier by working with you to understand the types of services we offer and how to participate in your own care. This way, you can feel less overwhelmed and more in control of your health.
We can also help eligible clients apply for medical assistance programs offered through Medicaid. We offer information to help providers coordinate care, prevent hospital readmissions, and improve the member’s health outcomes.
We can provide information about various types of home care and community resources, as well as make arrangements for:
- Short-term support services at home – such as a visiting nurse or aide, physical, occupational or speech therapy, medical social services, medical equipment and supplies, Home Delivered Meals, a companion, or transportation.
- Short-term nursing home care to prepare you for living independently at home.
- Information for you and your doctor regarding homecare policies, financial arrangements, insurance coverage, and other issues.
- Continuous follow-up to assure that you are actually receiving the services we have arranged.
- Supportive counseling to help you cope with changes in your life resulting from your illness.
